When is prostate surgery indicated?

Prostate surgery is indicated when the prostate is larger than 50 gm and the flow if the urine has reduced below 10ml/sec, and the residual urine has increase beyond150ml. Some patients have difficulty in holding urine these patients also benefit from prostate surgery.

Generally three weeks of active rest at home is enough for complete recovery from a laser prostate surgery. During this period any heavy weight lifting is prohibited like lifting of heavy bucket of water. Also the patient should avoid straining at stools for which they are generally advised to take lady finger and isobgul husk...

Can heart patients be treated with laser prostate surgery?

Since prostate is a disease of the elderly male therefore many of the patients have cardiac issues. Prostate laser surgery is much safer than electroresection of the prostate by TURP. · Lesser bleed therefore less need to stop blood thinners · Lesser cardiac strain since less water is absorbed during laser surgery · Safe in patients on pacemaker since there is no electrical interference So in cardiac cases consult Dr Avishek Mukherjee MS MCh for further care.

What is Laser Prostate Surgery ?

Laser prostate surgery does not use any electrical current and therefore is safer in patients. The bleed is less and therefore the infection rate is less. The patient has a faster recovery. In cardiac cases on blood thinners the patient can be operated safely with little need to stop the blood thinner. In patients with pacemakers the turp electrical surgery can interfere with the pacemaker, since laser does not interfere with the pacemaker, the surgery is safer in patients with pacemakers. Even in patient with poor cardiac ejection fraction the laser does not increase the load on the heart and therefore is safer than TURP.

Initially the bladder masses can be detected by ultrsonography but the best test to detect the bladder cancer is cystoscopy to evaluate the bladder. In case of superficial bladder cancer the treatment is laser resection of the bladder tumor. However these tumors will tend to recur and therefore the patient is kept under cystoscopic surveillance for atleast ten years. The patient however has excellent quality of life and can lead a perfectly normal professional and domestic life. In case the tumor is muscle invasive then we need to remove the entire bladder and the prostate by radical laparoscopic surgery and then we reconstruct a new passage for the urine to pass which is called ilial conduit or a neo urinary bladder is constructed. But hernias can also develop in the umbilicus and also when a surgical scar hasnt healed properly then a incisional hernia can develop.

Hernias are initially discovered by the patient as a soft ball like bulge which is more evident when the patient is standing or straining at stools or while coughing. However in later cases there may be pain and in evem later cases the intestine camln get obstructed and strangulated within the hernia, this is a life threatening complication of hernia.

Hernias are always best treated by surgery. There is no medical management. Under no circumstances should a hernia be tightly compressed by a hard material since the intestinal contents of the hernia may be damaged. The latest care today is robotic surgical repair of the hernia. This is performed by the assistance of a surgical robot.
